Instructions For Application for Board Certification

General Information / Instructions

Upon submitting your Email requesting the Application for Board Certification by the American Board of Maritime Medicine (ABMM) you will be emailed the Application.

The Application for Board Certification should be TYPED or PRINTED for submission purposes.

The Application is a fillable PDF form. Please use Adobe Acrobat Reader to fill this form, then email the completed form to Secretary@Maritime-Medicine.com. Be Sure to save a copy for your own files.

The NOTES section (at the end of Application) may be used if more space is required under any Section of the application. If you do use the Notes Section, Please designate the corresponding Section Number of the Application for which you are providing additional information.

All materials received by the ABMM shall become the property of the ABMM and no originals nor copies of such documents shall be provided to any requesting party, including the supplying party.

You Agree that Typing or Printing your name in any location required in this Application shall be accepted as your signature for legal purposes.

Please do not use abbreviations when completing this application.

General Information regarding Adobe

Three (3) common user issues which arise with Adobe include the following:

  1. When you Receive the Email with the Adobe fillable Application Form - make sure you download the Application FROM THE EMAIL TO YOUR COMPUTER – THEN - Launch Adobe Acrobat Reader to open the file. Do NOT open the attachment directly from the email you received; otherwise, your edits may not be saved.
  2. Most individuals have the free Adobe Acrobat Reader already. But some individuals use PDF Viewer and other PDF programs. This is intended to work with Adobe Acrobat Reader. If Adobe Acrobat Reader is not the default reader on your computer, you may need to install Adobe Acrobat Reader.
  3. Adobe Acrobat Reader is free at http://get.adobe.com/reader/. Before installing on the computer, notice that there are may be checkboxes to install Adobe Photo Express and MacAfee (that may be Already “checked” by default); you may want to “uncheck” those boxes before down-loading.

General Information Regarding the Sections of the Application:

Section 1: This Section simply re-emphasizes certain instructions.

Section 2: This Section is just a reminder of the required documents which are necessary to be submitted along with the Application. They may be emailed separately to Secretary@maritime-medicine.com – be sure you clearly state Your Name in the Email and Which Document(s) you are sending. The Documents may be sent as a combined PDF format if you desire; or as individual PDFs.

PLEASE NOTE: That only a Copy of your current professional licensure (e.g., MD, RN, Paramedic, etc.) will be required to establish eligibility to take the Board Certification.

HOWEVER, an official CERTIFIED Copy of your current professional licensure (e.g., MD, RN, Paramedic, etc.) will be required in order to receive your American Board of Maritime Medicine Board Certificate upon successful completion of the ABMM Examination.

For purposes of Board Certification in Maritime Public Health, for qualification purposes, a simple “copy” of a form of Licensure in Public Health (if applicable) shall be sufficient, Or, where a public health license is not available, a copy of the Master in Public Health degree (MPH) (or a higher degree in Public Health) shall serve as sufficient. A “CERTIFIED” copy of either the Public Health Licensure or MPH or higher degree will however be required prior to the American Board of Maritime Medicine Board Certificate in Maritime Public Health being issued upon successful completion of the ABMM Examination.

Section 3: Simply Check the box to Identify which Board Certification you are applying for.

Section 4: This Section simply requests your basic contact information – Please be sure it is Complete and Accurate.

Section 5: Provide the information requested; being SURE to provide the Registrar or Program Coordinator’s Email where requested. If any part of this Section does not apply to you, leave that portion of the Application Blank.

Section 6: Simply provide the Requested information as may be applicable to you.

Section 7: Simply provide the Requested information as may be applicable to you.

Section 8: Simply provide the Requested information as may be applicable to you.

Section 9: Simply provide the Requested information as may be applicable to you. It is Your responsibility to be sure that your PEER References actually send their Letters of Recommendation to the Secretary@maritime-medicine.com.

Section 10: Simply provide the Requested information as may be applicable to you. Please be Sure to provide the Supervisor’s Email (or Contact person who may have now replaced your previous Supervisor).

Section 11: Simply provide the Requested information as may be applicable to you.

Section 12: Please submit the requested CME documentation as an Attachment to: Secretary@maritime-medicine.com.

Section 13: Simply provide the Requested information – this applies to ALL TYPES of Board Certifications being requested. These clinical summaries should indicate the complexity of your clinical management of the type of Maritime Issue you were involved with, thereby reflecting your complex decision-making processes.

Section 14: Simply Truthfully Answer the Attestation Questions as “Yes” or “No.”

Section 15: Please Read Carefully and provide your Signature (by Typing in Your Name) if you Agree.
